Concourse Rehab and Nursing Center has 240 certified beds that have been approved by the federal government to participate in Medicare and Medicaid. Short-stay residents. For short stay residents (100 days or less), Concourse received an overall rating of 2 stars. The rating is based on a review of the following factors:

- Percentage who have had an outpatient emergency department visit
- Percentage who got antipsychotic medication for the first time
- Percentage of residents with new or worsened pressure injuries (bedsores)
- Percentage who improved in their ability to independently move around
- Percentage who needed and received a flu shot for the current flu season
- Percentage who needed and received a pneumonia vaccine
- Percentage of residents whose medications were reviewed and who received follow-up care when medication issues were identified
- Percentage of SNF residents who seriously injured from a fall during their SNF stay
- Percentage of SNF residents whose functional abilities were assessed and functional goals were included in their treatment plan
- Percentage of residents who are at or above an expected ability to care for themselves at discharge
- Percentage of residents who are at or above an expected ability to move around at discharge
- Change in residents' ability to care for themselves
- Change in residents' ability to move around
- Percentage of SNF healthcare personnel who completed COVID-19 primary vaccination series
- Rate of successful return to home or community from a SNF
- Rate of potentially preventable hospital readmissions 30 days after discharge from a SNF
- Percentage of infections patients got during their SNF stay that resulted in hospitalization
- Medicare Spending Per Beneficiary (MSPB) for residents in SNFs
Long-stay residents. Concourse’s rating for long-stay residents (over 100 days) is 5 stars which is a “much above average” rating. The rating is based on the following factors.
- Number of hospitalizations per 1,000 long-stay resident days
- Number of outpatient emergency department visits per 1,000 long-stay resident days
- Percentage of residents who got an antipsychotic medication
- Percentage of residents who suffered serious injury from falling
- Percentage of high-risk residents with bedsores/pressure ulcers
- Percentage of residents with a urinary tract infection
- Percentage of residents who have or had a catheter inserted and left in their bladder
- Percentage of residents whose need for help with daily activities has increased
- Flu & pneumonia prevention measures - Long-stay residents
- Percentage of residents who needed and got a flu shot for the current flu season
- Percentage of residents who needed and got a vaccine to prevent pneumonia
- Percentage of residents who were physically restrained
- Percentage of low-risk residents who lose control of their bowels or bladder
- Percentage of residents who lose too much weight
- Percentage of residents who have symptoms of depression
- Percentage of residents who got an antianxiety or hypnotic medication

Fire Safety Inspections & Emergency Preparedness
Nursing homes have regular fire safety inspections and they are required to have an emergency preparedness plan, based off an all-hazards risk approach. Concourse’s most recent standard fire safety inspection was on March 30, 2022. It received 5 citations.
