Located in Whitestone, New York, Bridge View Nursing Home (“Bridge View”) has 200 certified beds that have been approved by the federal government to participate in Medicare and Medicaid. The Centers for Medicare & Medicaid Services (CMS) gives each nursing home an overall rating on a scale of 1 to 5 stars. • Percentage of long-stay residents who got an antipsychotic medication
• Percentage of long-stay residents experiencing one or more falls with major injury
• Percentage of long-stay high-risk residents with pressure ulcers
• Percentage of long-stay residents with a urinary tract infection
• Percentage of long-stay residents who have or had a catheter inserted and left in their bladder
• Percentage of long-stay residents whose need for help with daily activities has increased

On January 31, 2022, Bridge View was fined $663. However, in the last 3 years it never had a Medicare payment denials.
