Fieldston Lodge Care Center

Fieldston Lodge Care Center (“Fieldston”) has 200 certified beds that have been approved by the federal government to participate in Medicare and Medicaid. It is not in a Continuing Care Retirement Community and is not in a hospital. It has a Resident and Family Council to facilitate communications with the staff. The ownership of the facility is a for profit corporation doing business under the legal business name of Fieldston Lodge Care Center.

The Centers for Medicare & Medicaid Services (CMS) gives each nursing home an overall rating on a scale of 1 to 5 stars. The overall rating* for Fieldston is 1 star which is an “much below average” rating.

The CMS also assigns a star rating in each of 3 categories: health inspections, staffing, and quality of resident care measures.

Health Inspections

Approximately once a year, New York State inspectors inspect nursing homes to make sure that they remain in compliance with the federal Medicare and Medicaid regulations. Fieldston’s most recent inspection was on March 1, 2022. As a result of the inspection, Fieldston was given a 1 star/much below average rating. It was issued 11 health citations, indicating that federal regulations had been violated.

Staffing

Fieldston received an “below average” rating (2 stars) for staffing. This rating was based on the totally number of nurse staff hours per resident per day, the registered nurse hours per resident per day, the LPN/LVN hours per resident per day, and the nurse aide hours per resident per day. It was also based on RN hours per resident per day and staffing turnover.

Quality of Resident Care

When it comes to quality measures, Fieldston received a 3 star rating, which is an average rating. The rating is based on quality of care for short term residents- those who spent 100 days or less at the facility or those covered under the Medicare Part A Skilled Nursing Facility benefit and on quality of care of long-term residents- those who spent over 100 days in a nursing home.

Short-stay residents. For short stay residents, Fieldston received an overall rating of 2 stars. The rating is based on a review of the following factors:

  • Percentage of short-stay residents who were re-hospitalized after a nursing home admission
  • Percentage of short-stay residents who have had an outpatient emergency department visit
  • Percentage of short-stay residents who got antipsychotic medication for the first time
  • Percentage of residents with pressure ulcers/pressure injuries that are new or worsened
  • Percentage of short-stay residents who improved in their ability to move around on their own
  • Flu & pneumonia prevention measures - Short-stay residents
  • Percentage of short-stay residents who needed and got a flu shot for the current flu season
  • Percentage of short-stay residents who needed and got a vaccine to prevent pneumonia
  • These measures are part of the Skilled Nursing Facility (SNF) Quality Reporting Program (QRP) and report information on residents who get skilled nursing services under their Medicare Part A benefit.
  • Percentage of residents whose medications were reviewed and who received follow-up care when medication issues were identified
  • Percentage of SNF residents who experience one or more falls with major injury during their SNF stay
  • Percentage of SNF residents whose functional abilities were assessed and functional goals were included in their treatment plan
  • Percentage of residents who are at or above an expected ability to care for themselves at discharge
  • Percentage of residents who are at or above an expected ability to move around at discharge
  • Change in residents' ability to care for themselves
  • Change in residents' ability to move around
  • Percentage of SNF healthcare personnel who completed COVID-19 primary vaccination series
  • Rate of successful return to home or community from a SNF
  • Rate of potentially preventable hospital readmissions 30 days after discharge from a SNF
  • Percentage of infections patients got during their SNF stay that resulted in hospitalization
  • Medicare Spending Per Beneficiary (MSPB) for residents in SNFs

Long-stay residents. Fieldston’s rating for long-stay residents is 5 stars which is a “much above average” rating. The rating is based on the following factors.

  • Number of hospitalizations per 1,000 long-stay resident days
  • Number of outpatient emergency department visits per 1,000 long-stay resident days
  • Percentage of long-stay residents who got an antipsychotic medication
  • Percentage of long-stay residents experiencing one or more falls with major injury
  • Percentage of long-stay high-risk residents with pressure ulcers
  • Percentage of long-stay residents with a urinary tract infection
  • Percentage of long-stay residents who have or had a catheter inserted and left in their bladder
  • Percentage of long-stay residents whose need for help with daily activities has increased
  • Flu & pneumonia prevention measures - Long-stay residents
  • Percentage of long-stay residents who needed and got a flu shot for the current flu season
  • Percentage of long-stay residents who needed and got a vaccine to prevent pneumonia
  • Percentage of long-stay residents who were physically restrained
  • Percentage of long-stay low-risk residents who lose control of their bowels or bladder
  • Percentage of long-stay residents who lose too much weight
  • Percentage of long-stay residents who have symptoms of depression
  • Percentage of long-stay residents who got an antianxiety or hypnotic medication

  • Residents who completed primary vaccination series 79.4% (lower than state and national averages)
  • Residents who are up-to-date on their vaccines 0% (lower than state and national averages)
  • Staff who completed primary vaccination series 100% (higher than state and national averages)
  • Staff who are up-to-date on their vaccines 2.7% (lower than state and national averages)

In the last 3 years Fieldston received one fine of $5,000. It did not have any Medicare payment denials in the last 3 years.
